Home Support Privacy Policy

1. Introduction

Care Connection Home Support ("Care Connection," "we," "us," or "our") is a Maryland-licensed Residential Service Agency (RSA Level 3) providing personal care services to clients in their homes. This Privacy Policy describes how we collect, use, disclose, and safeguard your information. We are committed to protecting your personal and health information in accordance with HIPAA, Maryland Office of Health Care Quality (OHCQ) regulations, and applicable law.


2. Information We Collect

Information collected during intake and service:

• Name, address, phone, date of birth, demographic information

• Emergency contacts and family/authorized representatives

• Medical history, diagnoses, medications, allergies

• Functional assessment and care plan

• Long-term care insurance, Medicaid, VA, or other payer information

• Authorized signers for plan of care and billing

• Service preferences and cultural/dietary needs

• Visit records, caregiver notes, and communication logs

Information from website visitors:

• IP address, browser data, cookies

• Contact form submissions


3. How We Use Your Information

We use your information to:

• Provide personal care, companionship, and home support services

• Coordinate with physicians, family, and other care providers

• Schedule caregivers and document services

• Bill payers and clients

• Comply with OHCQ licensing requirements

• Provide care plan updates and family communications

• Improve quality of services through internal review


4. HIPAA Notice of Privacy Practices

Care Connection is a HIPAA-covered entity. Our full Notice of Privacy Practices (NPP) is provided at intake and is available on request.


5. Maryland Client Rights

As a Maryland RSA Level 3 client, you have specific rights including:

• The right to be treated with respect and dignity

• The right to privacy and confidentiality

• The right to participate in your care plan

• The right to refuse services

• The right to receive a copy of these rights at intake

• The right to file a complaint without retaliation

• The right to have your family or authorized representative involved

• The right to be informed in advance of any changes in service or cost

A complete written copy of your Client Rights is provided at the start of services.


6. How We Share Your Information

We share information only as necessary and as permitted by law:

• Your physician and clinical team — for care coordination

• Family members and authorized representatives — per your authorizations

• Long-term care insurers and payers — for verification and billing

• OHCQ and licensing authorities — as required for licensing and inspections

• Business associates — under signed BAAs (HR for Health, Gusto, QuickBooks, WellSky, Spruce,

RingCentral, HubSpot, MailChimp)

• Affiliated entities — including Woundworx and The Rest Rx when clinically relevant; you may choose

alternative providers

• Legal authorities — in response to subpoenas, court orders, abuse reporting requirements, or other

legal obligations


We do not sell your information.


7. Data Security

We use administrative, physical, and technical safeguards including secure scheduling systems (WellSky), HIPAA-compliant communications (Spruce), encrypted record-keeping, and ongoing staff training.


8. Data Retention

We retain client records in accordance with Maryland OHCQ requirements and applicable state and federal law.


9. Children's Privacy

Our services may be provided to clients under 18 with parent or legal guardian consent and physician involvement. We collect and use the information of minor clients only as needed to provide care and in compliance with applicable law.


10. Mandatory Reporting

Care Connection caregivers and clinical staff are mandatory reporters under Maryland law. We are required to report suspected abuse, neglect, or exploitation of vulnerable adults to Adult Protective Services (or to law enforcement) regardless of your wishes.

12. Changes to This Policy

We may update this Privacy Policy. The "Last Updated" date reflects the most recent revision.
