1. Acceptance of Terms

By engaging Care Connection Home Support ("Care Connection") for services or by using careconnectionhomesupport.com, you agree to these Terms of Service.

2. About Care Connection Home Support

Care Connection Home Support is a Maryland-licensed Residential Service Agency (RSA Level 3) providing non-medical personal care services to clients in their homes. Services may include personal care, companionship, light housekeeping, meal preparation, medication reminders (non-administration), mobility support, and similar services within the legal scope of personal care.

3. Scope of Services

Care Connection caregivers provide non-medical personal care only. Care Connection caregivers do not:

• Administer medications (we may provide reminders only)

• Perform clinical assessments, diagnoses, or treatments

• Perform skilled nursing services (those require a separate licensed home health agency)

• Make clinical decisions

• Provide medical advice

If you require skilled nursing or medical services, we will refer you to a licensed home health agency or your physician.

4. Client Rights

You will receive a written copy of your Client Rights at the start of service. These rights are guaranteed under Maryland law and include the right to be treated with respect, the right to privacy, the right to participate in your care plan, the right to refuse services, and the right to file a complaint without retaliation.

5. Service Agreement and Plan of Care

Before services begin, you (or your authorized representative) will sign a Service Agreement that includes:

• Description of services

• Schedule and rates

• Payment terms

• Caregiver qualifications and responsibilities

• Plan of Care developed with input from you and your physician (where applicable)

• Emergency and after-hours procedures

You may modify your Plan of Care at any time by contacting our office.

6. Payment and Billing

Care Connection accepts payment from:

• Private pay — credit card, check, or ACH

• Long-term care insurance — we coordinate with most major LTC insurers

• Maryland Medicaid Community Personal Assistance Services (when approved)

• VA Aid and Attendance (when approved)

• CareScout / Genworth network (when applicable)

You are responsible for any portion of fees not covered by insurance and for prompt payment of self-pay fees per the Service Agreement.

7. Cancellations and Schedule Changes

We require 24 hours notice for cancellation of scheduled visits. Cancellations made less than 24 hours in advance may be billed at the regular rate. Repeated cancellations or no-shows may result in service modifications or termination.

8. Mandatory Reporting

Care Connection caregivers are mandatory reporters under Maryland law. We are required to report suspected abuse, neglect, exploitation, or unsafe conditions affecting clients to Adult Protective Services or law enforcement, regardless of client wishes.

9. Caregiver Conduct and Termination

Our caregivers are screened, trained, and bound by professional codes of conduct. We do not tolerate abuse, theft, harassment, or unsafe behavior — by caregivers or by clients. Either party may terminate the service relationship at any time as outlined in your Service Agreement.

10. Affiliated Entities

Care Connection is part of a portfolio of healthcare-related entities, including Woundworx (affiliated mobile wound care clinical practice) and The Rest Rx (affiliated DMEPOS supplier). When referrals between affiliated entities occur, we disclose the affiliation and you may choose alternative providers at any time.

11. Privacy

Your information is collected, used, and protected as described in our Privacy Policy, which is incorporated into these Terms by reference.

12. Website Use

You may use careconnectionhomesupport.com for lawful purposes. You may not attempt unauthorized access to the site, use automated tools to scrape data, or transmit harmful code or interfere with site operation.

13. No Medical Advice

The information on this website is for general informational purposes only and does not constitute medical or legal advice. Care decisions should be made in consultation with qualified healthcare professionals. In a medical emergency, call 911.

14. Intellectual Property

All content on the website is the property of Care Connection or its content suppliers and is protected by copyright law.

15. Disclaimer of Warranties; Limitation of Liability

Our website and services are provided as described in your Service Agreement. We make no warranties beyond those required by law or specified in the Service Agreement. Nothing limits our liability for matters that cannot be limited under applicable law.

16. Governing Law and Dispute Resolution

These Terms are governed by the laws of the State of Maryland. Disputes shall be resolved in the state or federal courts of Maryland.

17. Changes to These Terms

We may revise these Terms. We will provide notice of material changes that affect existing clients.
